Best and fastest... always an opinion and the default settings are determined by the developers to be best overall.
However, some of my comments:
I think that rootkit scans are important, but it will add a bit of time, not much in my opinion. If you do not choose to scan for rootkits on a regular basis, you should include it in one of your full scans periodically. I keep this on because rootkits are a real threat.
You should scan for all archives periodically, especially if you do a lot of downloading. This is probably the biggest increase to full scan time. I do it about once a quarter, but I do not do a lot of downloading.
Lowering the scan priority is only important if you work on the PC while it is scanning. If you turn on the scan and walk away, you do not need to check this item.
As far as the file exclusions, these are common files like outlook, outlook express that the developers have deemed to be not a risk. I left mine as is.
Creating a retore point is always a good thing and does not take that long to do.
I am sure that someone will put another spin on the settings for you.
