I've seen this before. Usually, when surfing a site that serves up images by way of a script. Sites like Deskmod.com</font color=red> (http://www.deskmod.com/><font) used to (they may still, I dunno) do this all the time.
The best way I found to get around it is one of two way. One, right-click the image and copy the URL of the image (if it's not the location of a script) and paste it into your address bar. Two, just refresh the page.
There's no real fool-proof method I've found to get around this.

Have seen it written that clean out/emptying Temporary Internet Files will restore the choices.

Hally, Bert has it.
It's almost always a full Temp Internet cache.
